Niu Xi (Achyranthes / Cyathula Root, the dried root of Achyranthes bidentata [Huai Niu Xi] or Cyathula officinalis [Chuan Niu Xi]) is one of the most functionally comprehensive herbs in the TCM repertoire: it simultaneously activates Blood, supplements Liver-Kidney, unblocks urinary passages, and directs Qi-Blood-Fire downward. This multi-directional action with a single directional pharmacological character — xing shan xia xing (“nature inclined to descend”) — is what makes Niu Xi pharmacologically unique. TCM properties: Bitter, sweet, sour, neutral; enters Liver and Kidney channels. Bitter drains and moves (supports Blood-Stasis dissolving and descending); sweet supplements (supports Liver-Kidney nourishing); sour astrings and enters Liver channel; neutral prevents thermal excess. The defining pharmacological character: xing shan xia xing (“nature inclined to descend”) — this is the single most clinically important property of Niu Xi. All five core actions share this descending directional character: Blood-Stasis dissolving downward, supplementing Liver-Kidney of the lower body, draining urinary stagnation downward, directing ascending Fire downward, unblocking lower-limb joints.
Three processing forms:
- Raw (sheng): strongest Stasis-expelling, channel-unblocking, fluid-draining, and downward-guiding action; used for Blood-Stasis, urinary disorders, and upper-body pathology requiring downward guidance
- Wine-processed (jiu): enhances Blood-activating, channel-unblocking, and sinew-bone strengthening action; wine carries the active compounds into the channels
- Salt-processed (yan): guides the herb into the Kidney channel (yin yao ru shen); enhances Liver-Kidney supplementing and sinew-bone strengthening

III. Five Core Actions

1. Expel Stasis and unblock channels — gynaecological Blood-Stasis:
Activates Blood and dissolves Stasis; unblocks and promotes menstruation. Target patterns: Blood-Stasis obstructing the uterine vessels producing amenorrhoea, dysmenorrhoea, post-partum lochia retention with Blood-Stasis abdominal pain, and failure of placenta to expel (bao yi bu xia). The primary herb for gynaecological Blood-Stasis. Pairs with Dang Gui, Tao Ren, Hong Hua, and Chuan Xiong (Blood-Stasis gynaecological formula framework). Chuan Niu Xi is preferred for this indication.
2. Supplement Liver-Kidney and strengthen sinew-bone:
Supplements Liver-Kidney Essence-Blood and strengthens the lumbar-knee sinew-bone complex. Target patterns: Liver-Kidney insufficiency producing lumbar-knee weakness and aching, lower-limb atrophy and weakness (wei bi), osteoporosis. The essential herb for elderly lumbar-knee preservation. Pairs with Du Zhong, Xu Duan, Sang Ji Sheng, and Gou Qi Zi (Huai Niu Xi preferred; salt-processed form preferred for this action). Modern pharmacology: β-ecdysterone promotes osteoblast activity, increases bone mineral density — the molecular basis for the anti-osteoporosis action.
3. Promote urination and unblock lin-urinary obstruction:
Unblocks the water passages and promotes urination; relieves lin-syndrome urinary obstruction and pain. Target patterns: Heat-lin (burning painful urination), Blood-lin (blood in urine with burning pain), oedema with urinary difficulty, small-intestine Fire generating short burning urination. Pairs with Che Qian Zi, Hua Shi, Ze Xie, and Qu Mai (Niu Xi Tang framework from Qian Jin Fang). Raw form preferred; the downward-draining character amplifies the urinary passage-opening action.
4. Guide Blood and Fire downward:
Directs ascending Fire and flooding Blood downward; the most distinctive systemic action in TCM herbology. Target patterns: upper-Heat lower-Cold and deficiency-Fire uprising patterns producing: head pain and dizziness, flushed face and red eyes, toothache and gum swelling, mouth sores and throat swelling, haematemesis and epistaxis from Blood-Heat rising. In these patterns, Blood and Fire are pathologically ascending; Niu Xi’s descending nature provides the downward-directing vector. Modern application: hypertension with dizziness, head pain, and flushed face from Liver-Yang uprising — Niu Xi in Zhen Gan Xi Feng Tang acts as the primary “guiding” herb. 5. Unblock and benefit the joints:
Activates Blood, unblocks channels, relaxes sinew and benefits joint mobility. Improves Wind-Damp bi-syndrome joint pain, joint immobility and restriction, traumatic swelling and pain, lumbar-leg aching pain. Pairs with Ru Xiang, Mo Yao, Xu Duan, and Gu Sui Bu (Shu Jin Huo Xue Tang framework).

V. Five Classical Formulas

1. Du Huo Ji Sheng Tang “Angelica-Loranthus Decoction” (Qian Jin Fang)
Composition: Niu Xi · Du Huo · Sang Ji Sheng · Du Zhong · Xi Xin · Dang Gui and others. Action: dispel Wind-Damp, stop bi-pain, supplement Liver-Kidney, supplement Qi and Blood. Indication: Wind-Damp bi-syndrome, lumbar-knee Cold-pain, lower-limb numbness, joint immobility. Niu Xi’s dual role: supplements Liver-Kidney (with Du Zhong, Sang Ji Sheng) while simultaneously unblocking channels and directing to the lower body.
2. Xue Fu Zhu Yu Tang “Driving Stasis from the Blood Mansion Decoction” (Yi Lin Gai Cuo)
Composition: Niu Xi · Tao Ren · Hong Hua · Dang Gui · Sheng Di · Chai Hu and others. Action: activate Blood and dissolve Stasis, move Qi and settle pain. Indication: Blood-Stasis chest pain, headache, insomnia, Blood-Stasis dysmenorrhoea. Niu Xi’s role: Blood-Stasis-dissolving component that also directs the formula’s action downward; ensures Stasis is cleared from the lower body and prevents the ascending tendency.
3. Zhen Gan Xi Feng Tang “Settle the Liver and Extinguish Wind Decoction” (Yi Xue Zhong Zhong Can Xi Lu)
Composition: Niu Xi · Dai Zhe Shi · Long Gu · Mu Li · Xuan Shen · Tian Dong and others. Action: settle and calm the Liver, extinguish Wind and subdue Yang. Indication: Liver-Yang uprising and Liver-Wind agitation — head dizziness and eye-dizziness, flushed face and red eyes, hypertension. Niu Xi’s role: the primary “guiding downward” drug; its downward-directing nature amplifies the formula’s ability to subdue ascending Liver-Yang. Zhang Xichun (the formula’s creator) used Niu Xi in a large dose (30g) precisely because its xing shan xia xing character is the key mechanism.
4. Niu Xi Tang (Qian Jin Fang)
Composition: Niu Xi · Dang Gui · Qu Mai · Hua Shi · Dong Kui Zi. Action: expel Stasis and unblock channels, promote urination and drain lin. Indication: placenta retention, urinary difficulty, lin-syndrome and urinary pain. Niu Xi’s dual Blood-activating and urinary passage-opening actions work simultaneously — appropriate for the pattern where post-partum Blood-Stasis combines with urinary difficulty.
5. Shu Jin Huo Xue Tang “Relax Sinew and Activate Blood Decoction”
Composition: Niu Xi · Xu Duan · Dang Gui · Ru Xiang · Mo Yao. Action: activate Blood and treat trauma, reduce swelling and settle pain. Indication: traumatic injury, lumbar-leg Blood-Stasis pain, joint sprain.

VIII. Dosage and Safety

Dosage: decoction 5–12g. Processing selection: Liver-Kidney supplementation — wine-processed or salt-processed Niu Xi (Huai preferred); expelling Stasis and draining lin — raw Niu Xi. Do not use excessively or long-term to avoid depleting constitutional Qi.
Contraindications:
- Pregnancy: ABSOLUTE CONTRAINDICATION. Blood-activating and Stasis-dissolving action combined with the downward-descending nature creates a high risk of inducing miscarriage. No exceptions
- Excessive menorrhagia (heavy menstrual bleeding): will worsen haemorrhage; contraindicated
- Middle-Qi sinking, Spleen-deficiency diarrhoea: the strongly descending nature may worsen diarrhoea and sinking; use with caution; may require Qi-uplifting herbs to be combined
- Lower-origin not consolidated (xia yuan bu gu), seminal emission, urinary incontinence: the draining-downward nature may worsen these patterns of failure to consolidate the lower-origin; use with caution
- Diet during treatment: avoid raw-cold and oily foods; prefer wine-processed or salt-processed form for Spleen-Stomach weaker patients
- Authenticity: genuine Niu Xi has characteristic 2–4 (Huai) or 4–7 (Chuan) concentric vascular bundle rings in the cross-section; these are the primary botanical identification feature
