A classical emergency formula from Wen Bing Tiao Bian for blazing heat sinking into the Pericardium, with high fever, clouded spirit and phlegm-heat blocking the orifices.
Therapeutic Principle
Traditionally selected when warm-pathogen heat has sunk into the Pericardium with phlegm blocking the orifices — high fever and clouded spirit — rather than residual chest heat, Heart fire with Yin deficiency, or Liver Yang rising with internal Wind.

Clinical Perspective
An Gong Niu Huang Wan, Tian Ma Gou Teng Yin, Huang Lian E Jiao Tang and Zhi Zi Chi Tang all involve heat disturbing the Heart-spirit, but at very different depths. Zhi Zi Chi Tang vents residual chest heat after the exterior has resolved. Huang Lian E Jiao Tang clears Heart fire while nourishing Yin-blood for deficiency-type insomnia. Tian Ma Gou Teng Yin calms rising Liver Yang and internal Wind without opening the orifices. An Gong Niu Huang Wan is reserved for blazing heat sinking into the Pericardium with phlegm blocking the orifices — high fever and clouded spirit — rather than milder vexation, Yin-deficiency insomnia, or Yang-rising dizziness.
