Da Cheng Qi Tang, also known as Major Order the Qi Decoction, is a powerful classical formula from Zhang Zhongjing’s Shang Han Lun (Treatise on Cold Damage). As the strongest member of the Cheng Qi Tang series, it is designed to vigorously purge heat accumulation from the intestines and drain excess heat.
This formula is primarily used for severe Yangming bowel excess patterns, addressing symptoms such as tidal fever, delirium, severe constipation with dry stools, hard abdominal fullness and pain that refuses pressure, a dry yellow or black tongue coating, and a deep excess pulse. It remains one of the most important purgative formulas in Traditional Chinese Medicine for clearing intense internal heat accumulation.
2. Acute Intestinal Obstruction
Da Cheng Qi Tang is remarkably effective for acute intestinal blockages. This condition happens when something blocks the intestines or the intestinal wall fails to move, leading to severe belly pain, bloating, vomiting, and a total stop in passing gas or stool. In Chinese medicine, this is a classic "Yang Ming Organ-Excess" pattern. 3. Acute Pancreatitis
During an attack of acute pancreatitis, the pancreas becomes inflamed, causing swelling and intense pain, often accompanied by severe bloating and vomiting. From a TCM perspective, this is usually seen as a buildup of "Internal Solid Heat" in the stomach and intestines. Da Cheng Qi Tang helps by flushing out this toxic heat, which reduces the inflammatory pressure on the pancreas, prevents the body from absorbing harmful toxins, and helps the digestive system recover faster.

For cases where a severe infection or high fever causes a patient to become delirious or lose consciousness—especially if they are also constipated and bloated—this formula is a vital tool. By purging the "Heat-Clumping" in the gut, it provides a "vent" for the internal fire to escape.
