Wu Mei Wan (乌梅丸), or Mume Pill, is a classical formula from Zhang Zhongjing’s Shang Han Lun (Treatise on Cold Damage). It belongs to the category of formulas that warm the viscera, drain Heat, calm roundworms, and treat Jueyin mixed Cold and Heat.
This formula addresses the Jueyin pattern of simultaneous Heat and Cold, classically associated with roundworm inversion and later widely used for chronic mixed-pattern diarrhea. Typical signs include intermittent vomiting or abdominal pain, vomiting of worms in the original indication, diarrhea that alternates or persists, a desire to eat yet vomiting after eating, cold limbs with internal Heat signs such as thirst or irritability, a pale or red tongue, and a wiry pulse.
Combining sour Wu Mei as the chief herb to calm worms and astringe the Intestines, Hua Jiao, Xi Xin, Gan Jiang, and Fu Zi to warm the viscera and dispel Cold, Huang Lian and Huang Bai to drain hidden Heat, and Ren Shen, Dang Gui, and Gui Zhi to tonify Qi and Blood and unblock the vessels, Wu Mei Wan simultaneously warms and clears, calms the worms, and restores the complex Jueyin dynamic of Heat above and Cold below.
2. Ascariasis Syncope (Roundworm Syncope)
Wu Mei Wan (Mume Pill) is the absolute classic prescription for treating roundworm syncope.

- This is typically accompanied by nausea, vomiting (sometimes even vomiting roundworms), and ice-cold hands and feet.
